To identify the associated factors (associated) with post-surgical complications due to appendectomy in pediatric patients at the Hospital de Apoyo II-2 Sullana, 2019-2020. Methods: A case-control, observational, analytical, cross-sectional, retrospective, analytical study was conducted; with secondary data collection (review of medical records). In this study, patients were divided into two groups: 40 cases (defined as patients with postoperative complications after appendectomy) and 40 controls (patients without postoperative complications), matched according to sex of the participant. Statistical analysis was performed in Stata v16 statistical software, and odd ratio was calculated for the associated factors. Results: Among the postoperative complications, the most common were adynamic ileus (22.5%), surgical site infection (20%) and seroma in the surgical wound (17.5%). Retrocecal position was more prevalent in cases than in controls (57.5% vs 25%, p=0.005), necrotic (50% vs 12.5%, p<0.001) and perforated (37.5% vs 17.5%) staging was more common in cases than in controls, similarly operative time was longer in cases (63.8 minutes vs 46.98 minutes, p<0.001). Delayed surgery occurred more frequently among cases than controls (42.5% vs 20%, p=0.03). Conclusions: Factors associated with postoperative complications after appendectomy were pelvic (OR: 3.09, 95%CI: 2.01-4.37) and retrocecal (OR: 4.35, 95%CI: 2.03-5.66) position of the appendix, perforated appendicitis (OR: 2. 39, 95%CI: 1.09-3.21) and necrotic (OR: 4.93, 95%CI: 2.03-5.66), surgeon experience greater than 10 years (OR: 0.11, 95%CI: 0.02-0.65) and delay of appendectomy by more than 24 hours (OR: 3.41, 95%CI: 2.82-10.41)
