BACKGROUND: Although the protective factor of exclusive breastfeeding (EBF) against acute respiratory infections (ARI) in children has been reported, there are few updated population studies, developed in poor countries that have considered interesting confounding variables under a statistical analysis that includes the sampling complexity. OBJECTIVE: To determine whether there is an association between EBF and the prevalence of ARI in children under 36 months, considering the potential role of confounding variables. MATERIALS AND METHODS: Secondary analysis of the Demographic and Family Health Survey (ENDES) of 2019 that included information from 3 839 children obtained after a multistage sampling. The complexity of the sampling allowing the calculation of crude and adjusted prevalence ratios. RESULTS: 12.9% of the children presented ARI and the prevalence of EBF was 39.8%. In the bivariate analysis, an association was found between ARI with age (p = 0.040) and area of residence (p = 0.033); the latter being statistically significant in the regression analysis (RRc = 0.73; 95% CI: 0.55 - 0.97; p = 0.033 and RRa = 0.74; 95% IC: 0.55 - 0.97; p = 0.033). In the crude and adjusted analysis of the regressions, there was no association between the main variables (RRc = 0.97; 95% CI: 0.67 - 1.38; p = 0.860 and RRa= 0.98; 95% IC: 0.69 - 1.40, p = 0.936, respectively). CONCLUSION: No significant association was found between EBF and ARI.
