Introduction: The influenza pandemic caused the death of 50 million people worldwide during its first year, becoming a public health problem and generating a health, academic, biopsychosocial and economic crisis. Therefore, these characteristics make it more interesting to analyze, in order to be able to face future pandemics of influenza and other viruses such as SARS-COV 2. Objective: To describe the demographic and geographic characteristics of mortality during the 1918-1920 influenza pandemic in the Lima, Loreto and Ica regions. Materials and methods: An ecological study that had the permission of the CDC-Peru to access the database of records of deaths during the years 1915-1922 in the regions of Lima, Loreto, Ica and La Libertad. The demographic characteristics of those who died from respiratory and non-respiratory causes, excess mortality and the identification of pandemic waves in the time series of each of the three regions were evaluated using R language software. Results: We worked with 7,036 records of deaths, in which we identified that the mortality curve was W-shaped, with young adults belonging to the female sex and children under 15 years of age being the most affected groups. In Lima there were 3 pandemic waves, with the one in January 1920 being the highest, Ica had 2 pandemic waves that did not have a great impact on mortality, while the Loreto region had a single pandemic wave that lasted 8 months. In addition, it was determined that in Lima and Ica the main cause of death was respiratory, while in Loreto it was gastrointestinal. It was found that in week 51 there was a high excess mortality in Ica and Lima, the latter region having the highest overall excess with 1594 (18.1%), while Loreto had an overall excess of 202 (52.7%). Conclusions: The analysis identified that the region farthest from the capital presented a higher mortality rate, that people of mixed ethnicity and those with trades were the most affected. Therefore, we must create public health strategies for future pandemics.
