Background: Hypertension is a frequent worldwide risk factor whereby prevalence, diagnosis, and treatment are constantly assessed. The objective of the present study was to examine the mortality among higher levels of blood pressure, not considered normal, under two different guidelines, JNC-7 y 2017 ACC/AHA. Methods: Data from PERU MIGRANT cohort. Independent variable was blood pressure levels under two guidelines definition (JNC-7 y 2017 ACC/AHA), and mortality follow-up was 10 years. Log Rank test, Kaplan-Meier Analysis and Cox regression models were used to assess correlates of survival, adjusted to confusing variables. Hazard Ratios (HR) and confidence intervals (CI) were determined. Results: A total of 989 participants, 52,6% women, mean age 48,0 (DE: 12,0), were included. The prevalence of hypertension was 31,3% and 16,0% comparing the 2017 ACC/AHA and JNC-7 guidelines, respectively. Mortality were higher among elevated blood pressure using either definitions. 2017 ACC/AHA classification, elevated blood pressure, HTA I and HTA II were associated HR 1,67(0,68-4,08); 2,83(1,23-6,52) and 5,11(2,47-10,60), respectively. Compared to normotension, hazard ratios of mortality were 2,14 (1,02-4,54), for pre-hypertension and 5,27(2,55-10,92) for hypertension, JNC-7. Conclusions: This study supports an association of elevated blood pressure with mortality under two guidelines.
