Introduction: Newborns, due to their developing immunity, rely on maternal passive immunity through transplacental and milk antibodies. During the COVID- 19 pandemic, the presence of specific IgA-S against SARS-CoV-2 in breast milk was confirmed; however, it has not been determined whether its concentration varies between the pandemic and post-pandemic periods. Objective: To describe and compare the presence and concentration of secretory IgA against the SARS- CoV-2 S antigen in breast milk collected during the pandemic and post-pandemic stages of COVID-19. Materials and Methods: A cross-sectional exploratory study analyzed the presence of specific IgA-S, using the ELISA method, in milk samples from 63 participants collected in 2021-2022 (pandemic) and 135 in 2023- 2024 (post-pandemic). Results: IgA antibodies were detected in 90.5% of colostrum in the pandemic group and 80.3% in the post-pandemic group; in mature milk, 80% and 82.5%, respectively. Antibody concentrations were higher in colostrum than in mature milk and comparable between periods: in colostrum, 16.9 ng/ml vs. 15.6 ng/ml (p = 0.3568), and in mature milk, 0.269 ng/ml vs. 0.172 ng/ml (p = 0.0783). In the post-pandemic period, vaccination during pregnancy was associated with higher antibody levels in colostrum compared to unvaccinated mothers (p = 0.019). Conclusions: IgA antibodies against SARS-CoV-2 are consistently present in breast milk, highlighting the protective role of colostrum for the newborn. Vaccination during pregnancy increases antibody levels, enhancing passive immunity in the infant.
