Resumen: Introducción: Actualmente vivimos en una realidad donde la pandemia del Covid-19
afecta a la población de diversas formas, si bien sabemos que la apendicitis aguda es una
emergencia quirúrgica, cuyo curso clínico y pronóstico depende del tiempo entre el inicio
de la enfermedad y las situaciones de emergencia. . Diagnóstico y tratamiento quirúrgico
definitivo. Por todo ello, el objetivo de este estudio fue determinar el impacto de la
pandemia en el manejo de la apendicitis aguda comparándolo con datos clínico
epidemiológicos antes y durante el primer año de pandemia. Objetivo: Determinar el
impacto de la pandemia por COVID 19 en el abordaje de la apendicitis aguda en IPRESS
III del cusco – 2020. Materiales y métodos: Es un estudio correlacional cualitativo
retrospectivo observacional transversal, en el que se evaluarán historias clínicas de los
pacientes con diagnóstico de apendicitis aguda atendidos en el servicio de cirugía general
del Hospital Nacional Adolfo Guevara Velasco del Cusco en el periodo de marzo a
diciembre del 2018 (periodo prepandemia) y de marzo a diciembre del 2020 (periodo de
pandemia). Conclusiones: Existe una disminución de los casos de apendicitis aguda en
el periodo de pandemia con un aumento de presentación de apendicitis complicada. El
tiempo de estancia prequirúrgica se ha prolongado durante el periodo de pandemia, esto
puede deberse a la aplicación de protocolos de bioseguridad o al retraso en la toma de
exámenes de laboratorio e imagen para el diagnóstico de COVID-19. El grado evolutivo
de presentación de apendicitis aguda descritos en el reporte operatorio y anatomía
patológica se ha modificado durante el periodo de pandemia, evidenciando mayor
presentación de los grados complicados. Existe un aumento en la presentación de
complicaciones post quirúrgicas y eventos adversos durante el periodo de pandemia,
asociados a una mayor estancia hospitalaria, reingreso hospitalario y necesidad de
reintervención.
Introduction: We currently live in a reality in which the covid-19 pandemic has affected the general population in multiple ways. As I see it, we know that acute appendicitis is a surgical emergency, whose clinical course and prognosis are determined. by the time between the onset of the disease, diagnosis in emergencies and definitive surgical management. For all these reasons, the present study aims to determine the impact of the pandemic on the approach to acute appendicitis by making a comparison with clinicalepidemiological data prior to the pandemic and during the first year of the pandemic. Objetives: To determine the impact of the COVID 19 pandemic on the approach to acute appendicitis in IPRESS III of Cusco - 2020. Materials and methods: It is a crosssectional observational retrospective qualitative correlational study, in which clinical records of patients with diagnosis of acute appendicitis treated in the general surgery service of the Adolfo Guevara Velasco National Hospital in Cusco from March to December 2018 (pre-pandemic period) and from March to December 2020 (pandemic period). Conclusions: There is a decrease in cases of acute appendicitis in the period of the pandemic with an increase in the presentation of complicated appendicitis. The presurgical stay time has been extended during the pandemic period, this may lead to the application of biosafety protocols or the withdrawal of laboratory tests and images for the diagnosis of COVID-19. The evolutionary degree of presentation of acute appendicitis described in the operative report and pathological anatomy has changed during the period of the pandemic, showing a greater presentation of complicated degrees. There is an increase in the presentation of post-surgical complications and adverse events during the pandemic period, associated with a longer hospital stay, hospital readmission and reintervention.
