Introduction: master formulation in pediatrics allows treatments to be personalized by adapting doses, pharmaceutical forms and excipients to the needs of children, improving the safety and effectiveness of the treatment. Given the lack of appropriate medications for this population, "off-label" prescriptions are frequently used, which increases risks and makes adherence difficult. Although compounding addresses these issues, it faces challenges such as a paucity of pharmacokinetic studies, insufficient regulation, and the use of inappropriate excipients. Method: 10 scientific articles published in the last five years on compounding formulation in pediatrics were analyzed, through qualitative synthesis and critical comparison. Results: pediatric master formulations guarantee biosafety by avoiding harmful excipients, improve acceptance through pleasant flavors and personalized presentations, and ensure stability with adequate pH control and selection of excipients, offering effective and safe treatments for children. Conclusion: the master formulation is an essential tool to make up for the lack of medications designed for children. Personalizing treatments, selecting appropriate excipients and developing age- appropriate formulations are key aspects to guarantee biosafety, treatment adherence and stability in pediatric formulations.
