Abstract: Introducción: la formulación magistral en pediatría permite personalizar tratamientos adaptando dosis, formas farmacéuticas y excipientes a las necesidades de los niños, mejorando la seguridad y eficacia del tratamiento. Ante la carencia de medicamentos adecuados para esta población, se recurre frecuentemente a la prescripción "off-label", lo que aumenta riesgos y dificulta la adherencia. Aunque la formulación magistral aborda estos problemas, enfrenta desafíos como la escasez de estudios farmacocinéticos, regulación insuficiente y el uso de excipientes inadecuados.
Método: se analizaron 10 artículos científicos publicados en los últimos cinco años sobre formulación magistral en pediatría, mediante síntesis cualitativa y comparación crítica.
Resultados: las formulaciones magistrales pediátricas garantizan la bioseguridad al evitar excipientes dañinos, mejoran la aceptación mediante sabores agradables y presentaciones personalizadas, y aseguran estabilidad con el control adecuado del pH y selección de excipientes, ofreciendo tratamientos efectivos y seguros para los niños.
Conclusión: la formulación magistral es una herramienta esencial para suplir la falta de medicamentos diseñados para niños. Personalizar tratamientos, seleccionar excipientes adecuados y desarrollar formulaciones apropiadas según la edad son aspectos clave para garantizar bioseguridad, adherencia al tratamiento y estabilidad en formulaciones pediátricas.

Given the lack of appropriate medications for this population, "off-label" prescriptions are frequently used, which increases risks and makes adherence difficult. Although compounding addresses these issues, it faces challenges such as a paucity of pharmacokinetic studies, insufficient regulation, and the use of inappropriate excipients. Method: 10 scientific articles published in the last five years on compounding formulation in pediatrics were analyzed, through qualitative synthesis and critical comparison. Results: pediatric master formulations guarantee biosafety by avoiding harmful excipients, improve acceptance through pleasant flavors and personalized presentations, and ensure stability with adequate pH control and selection of excipients, offering effective and safe treatments for children. Conclusion: the master formulation is an essential tool to make up for the lack of medications designed for children. Personalizing treatments, selecting appropriate excipients and developing age- appropriate formulations are key aspects to guarantee biosafety, treatment adherence and stability in pediatric formulations.
