The study seeks to provide information to redefine current protocols in the diagnosis and treatment plan in class III patients without growth. The objective of the present study is to evaluate the factors associated with the orthodontist's decision for orthodontic camouflage or orthognathic surgery in adult patients with class III skeletal malocclusion, Peru-2023. The methodology was analytical research level, prospective. An online inventory prepared in the Survey Monkey software with 16 closed questions and dichotomous – polytomous answers was used, which were sent to Orthodontic specialists. The results were obtained from a sample made up of 287 orthodontists, 175 men and 112 women, with 10 (3.5%) from 20 to 30 years old, 111 (38.7%) from 30 to 40 years old and 166 (57.8%) with more than 40 .years of age, of which orthodontic camouflage was chosen more frequently (63.4%) than orthognathic surgery (36.6%). In conclusion, according to the results analyzed with respect to the adjusted model, years of experience practicing the orthodontic specialty, training in surgery, cephalometric measurements related to the gonial angle and the presence of anterior crossbite were risk factors. The presence of the correctly erupted lower third molar together with the space provided by the tooth and occlusal stability are presented as protective factors against the decision.
