Introduction: Shortcomings in medical death certificates, including common errors, lack of medical training, and ethical obstacles, are revealed in the literature, highlighting the need to improve training and the medical death certification process in several countries, including Peru. Objective: To determine the quality of the completion of medical death certificates at Hospital II-2 Tarapoto, 2022. Material and methods: A descriptive and non-experimental methodology was used to evaluate a sample of 181 medical death certificates. Data collection was performed by documentary analysis of the certificates, identifying errors and omissions in the filling out, such as incorrect sequences of pathological events, lack of time interval, use of abbreviations, illegible writing, omission of information and other inconsistencies. The quality of the certificates was analyzed in terms of the presence of inconsistencies. Results: Several significant inconsistencies were highlighted, including missing time interval in 70.7% of cases, incorrect or unlikely sequences of pathologic events in 48.6%, poorly defined or specified basic causes of death in 38.1%, and errors related to pathologic states not linked to the main cause of death in 29.9%. There were also 2 certificates (1.1%) without the signature, fingerprint or stamp of the physician, all considered serious inconsistencies. In terms of quality, 50.3% were rated as poor and 35.9% as bad. Conclusion: Most of the medical death certificates presented a poor quality level.
