The present study aimed to determine the relationship between highly active antiretroviral treatment and adverse effects in the newborn of pregnant women attended at the Regional Hospital of Loreto, January 2019 - December 2021. This is a basic research with a quantitative approach, non-experimental, retrospective, correlational, cross-sectional design. The population consisted of 211 medical records with a calculated sample of 137, documentary analysis technique and analysis guide instrument. Results: The predominant socio-demographic characteristics were the followings: 73.7%, were mothers between 20 and 34 years old, 48.9%, have urban origin, 65%, have high school education, 67.2% are partners, 87.6% are homemakers. Obstetric and gynecological characteristics: 66.4% had > 2 different sexual partners < 6 months, 69.3% were multiparous and 46.7% had > 6 prenatal visits. HIV diagnosis was made and treatment started before pregnancy in 81%, during pregnancy in 15.4% and during labor in 3.6%. The adverse effects presented were: dystocic delivery (cesarean section) with 68.6%, anemia with 43.1%, preterm delivery with 23.4%, low birth weight with 15.3%, lymphopenia with 5.8%, thrombocytopenia with 4.4%, early fetal death and functional congenital malformation in 2.2%. There is a statistically significant relationship between prematurity (X2=18.694; p=0.000), anemia (X2=6.173; p=0.046), fetal death and functional congenital malformations respectively (X2=6.247; p=0.044) and dystocic birth (X2=11.211; p=0.004). It is concluded that there is a highly significant relationship between the treatment options received by the mother and the adverse effects present in the newborns (p<0.001).
