The objective of the research was to "determine the main factors associated with the non-compliance of the correct filling of the nursing notes in the medical service of the Amazonian Hospital of Yarinacocha, from July to December 2017". Descriptive, prospective, cross-sectional study; correlational The population was 7 nurses and 126 sheets of nursing notes, as instruments of data collection a questionnaire was applied and a checklist was applied, through direct observation and evaluation. The results were: the personal factors that limit the correct filling of the nursing notes are: the lack of knowledge of the PAE and SOAPIE (100%), the excess of work (57.1%) and the stressed emotional state (42, 9%); Among the institutional factors: the demand of patients (100%), deficient number of nurses (100%), non-updating (71.4%), lack of recognition (71.4%) and the lack of standards written (57.1%). Regarding the quality of the notes: 92% comply with filling the sheet correctly, according to content 79.4% does not adequately formulate the diagnoses, 47.6% does not use diagnostic labels of the NANDA, 65.1% does not adequately record the objective of the care plan. In conclusion: the personal factors are not associated with the failure to correctly fill the nursing notes p-value> 0.05 and a value X2Crit> X2Cal (95%), only the institutional factor supervisory actions are associated, p = 0.008 (p <0.05).
