Lack of adherence to drug therapy in hypertensive patients is a public health problem that constitutes an obstacle to blood pressure control. Every population has different realities and therefore the approach to solve it should be in accordance with these characteristics in order to avoid consequences such as strokes or heart attacks, as well as damage to other organs. The objective was "to determine the factors associated with adherence to pharmacological treatment of patients with arterial hypertension in the Rural Hospital of Nueva Cajamarca". The conducted research was of a correlational, cross-sectional type, with a sample of 80 patients attended in the outpatient clinic in the period of September 2022. For this purpose, a convenience sampling was carried out and a data collection form was applied, including the 4-item Morisky Green test. The SPSS v27 program was used for data processing. Results: The adherence rate was 47.5%. In the sociodemographic aspect, patients were predominantly female, between 45 and 59 years of age, with independent occupation, cohabitants and with primary education, with no statistical association found. In the factors related to the disease, 60% of the patients presented comorbidity and there was no difference between adherent and non-adherent patients; regarding the years of diagnosis, those with 1 to 4 years of disease predominated in the non-adherent group (57.1%) and in the adherent group those with 5 to 9 years (28.9%) and more than 10 years of disease (26.3%), finding a significant association (p= 0.02). In terms of therapeutic regimen, 63.2% of adherent patients had monotherapy and 28.39% had dual therapy; and as for frequency, 65.8% of adherent patients took their treatment 2 or more times a day, and there was no significant statistical association. The 65.8% of adherent patients had acceptable knowledge and 52.6% carried monthly controls with significant association (p =0.00 and p= 0.03) respectively. Conclusions: The following are associated in moderate intensity to adherence to antihypertensive pharmacological treatment: years of diagnosis, follow-up consultations and knowledge about the disease.
