Introduction: Hip fracture in older adults is the common cause of hospitalization, affecting their independence regarding basic activities. Objective: To determine the prognostic factors of unfavorable evolution, in patients older than 60 years operated on for hip fracture, at Hospital II-2 Tarapoto, from January 2020 to December 2022. Methods: Descriptive, retrospective study of patients over 60 years old who underwent surgery between January 2020 and December 2022. The population consisted of 161 postoperative hip fracture patients and was divided into 2 groups: one without complications (80) and the other with complications (81). For the statistics, graphs were made to evaluate the frequency of the univariables. For hypothesis testing, chi-square, odds ratio, 95% CI and multivariate linear logistic regression analysis were used for the significant variables. Statistical significance was p value < 0:05. Results: The age was 77.57 years, 63.6% were women, the most frequent age range was 60 to 69 years (47.8%), they had a stable partner (71.4%) and came from the urban area (56.5%). Post-surgical complications encountered were: constipation (40.0%), urinary retention (33.3%), urinary tract infection (16.2%), arrhythmias (12.4%), delirium (12.4%), acute renal failure (11.4%), surgical site infection (10.5%), COPD decompensation (6.70%). Multivariate analysis determined that factors associated with unfavorable evolution are: hemoglobin ≤ 10 g/ml (OR= 11.905; 95% CI 4.060-34.880; p=0.000) comorbidities ≥ 3 (OR= 2.484; 95% CI 1.080-5.710; p= 0.030) and surgical void ≥ 48 hours (OR= 2.640; 95% CI 1.382-5.044; p= 0.003). Conclusion: Prognostic factors for unfavorable evolution, in patients older than 60 years operated of hip fracture are hemoglobin ≤ 10 g/ml, comorbidities ≥ 3, and surgical void ≥ 48 hours.
