Introduction: HELLP syndrome represents a gestational hypertensive disorder, being a severe obstetric complication causing maternal death. The triad of hemolysis, increased liver enzymes and plateletopenia is its main characteristic. It develops in 0.1 to 0.2% of pregnancies. It is important because of its nonspecific clinical presentation, in addition to its fatal consequences without timely management. Objective: To determine the main epidemiological and clinical characteristics of patients with HELLP syndrome at Hospital II-2 Tarapoto, January 2018 - May 2023. Material and methods: Basic, descriptive, cross-sectional and retrospective research, with a population of 80 women and a sample of 68. Results: The age at presentation was 29.7 years, with a predominance of the 18 to 35 years range (58.82%), most of them with secondary education (44.12%), from rural areas (61.76%), and cohabiting marital status (72.06%). They had an adequate number (57.35%) of prenatal checkups, mostly 6 (29.41%). The majority were diagnosed as pregnant (60.29%), with a gestational age at the end of pregnancy of 35.3 weeks and reaching term in 52.94%. Similarly, the main clinical manifestations in most cases were: headache (31.45%), abdominal pain (29.56%), visual disturbances (15.09%), nausea (13.84%), vomiting (6.92%) and tinnitus (3.14%). More than 80% presented elevated BP ≥ 140/90 mmHg, with an average SBP 157.5 ± 19.1 mmHg, DBP 98.0 ± 12.1 mmHg. Among the risk factors with the highest percentage were: hypertensive disorder in previous pregnancy (31.8%), maternal age <18 - >35 years (28.4%), obesity (13.6%), 1st degree family history of preeclampsia (5.7%), pregestational diabetes mellitus (4.5%), chronic arterial hypertension (2.3%), current multiple pregnancy (1.1%) and chronic kidney disease (1.1%), however 10 patients (11.4%) had no identifiable risk factor. Class II was the most frequent according to the Mississippi classification. Regarding laboratory data, to confirm hemolysis, an average of 1712 IU/L and 2.8/dL of LDH and indirect bilirubin, respectively, were obtained. Liver alterations evidenced by GOT and GPT had an average of 198 IU/L and 287 IU/L, respectively, and platelets of 89853/mm3 on average. There were 2 deaths out of the total, which corresponds to a 2.94% frequency of death. Conclusion: Adequate reproductive age, high school education, rural origin, and cohabiting marital status are the epidemiological characteristics found in this study. On the other hand, the main clinical characteristics are: Number of adequate prenatal controls, pregnant condition at diagnosis, preterm gestation, clinical manifestations such as headache and abdominal pain, in addition, blood pressure more than 140/90mmHg, and risk factors such as hypertensive disorder in previous pregnancy, maternal age <18 - >35, and obesity. Class II is the most frequent type of HELLP syndrome. Laboratory data exceed normal values and reach the diagnostic range for HELLP syndrome. The frequency of death was 2 of 68 patients.
