The study analyzes the causes of deferral and rejection in blood donors at the Hospital Nacional Edgardo Rebagliati Martins during the COVID-19 pandemic, covering the period from March 2020 to August 2021. Out of a population of 49,997 donors, 24,312 (49%) were considered unsuitable, with 90% being temporarily deferred and 10% permanently rejected. The causes of exclusion were divided into biological, clinical, behavioral, and other factors, with a particular focus on pandemic-related factors such as COVID-19 diagnosis, exposure, and vaccination. An 8% increase in total deferrals was observed from 2020 to 2021, mainly due to COVID-19 diagnosis and the introduction of vaccination. Biological causes, especially low hematocrit, accounted for a significant proportion of deferrals. Additionally, deferrals due to exposure to the virus decreased by 70% in 2021. These findings highlight how the pandemic significantly impacted eligibility criteria for blood donation, emphasizing the need to adapt donation practices to emerging public health circumstances.
