Introduction: Multisystem Inflammatory Syndrome in children temporarily associated with COVID-19 (SIM-C) is a rare pathology and therefore little known; However, it is important to describe the clinical characteristics of SIM-C in the pediatric population of a high-altitude city such as Cusco city to carry out timely follow-up of cases. Objective: Describe the epidemiological and clinical characteristics of Multisystem Inflammatory Syndrome in pediatric patients with COVID-19 in hospitals of Cusco city, 2020-2022. Methods: Descriptive, cross-sectional and retrospective study. With a sample of 21 patients hospitalized in one of the 3 hospitals in Cusco that met SIM-C criteria. A data collection form was applied using the documentary review technique of medical records. Results: It shows a greater prevalence of the Kawasaki phenotype, with a wide age range, a slight predominance of the female sex. Prolonged fever, gastrointestinal and respiratory symptoms, together with tachycardia, tachypnea and oxygen desaturation, underscore the systemic involvement of the disease. Lymphopenia, neutrophilia, and thrombocytopenia are consistent with the inflammatory clinical profile of the disease. Interstitial infiltrates were seen on the chest x-ray and ground glass opacity on the tomography; Hepatomegaly and splenomegaly were also documented in ultrasound studies. Conclusions: This study shows a significant prevalence of the Kawasaki phenotype, with common symptoms such as fever, abdominal pain, diarrhea and respiratory distress. The clinical severity, reflected in oxygen desaturation and the need for hospitalization in the intensive care unit, highlights the seriousness of the disease.
