Introduction: Non-adherence to anemia treatment is the leading cause of therapeutic failure; it is a public health problem with serious economic, health and social repercussions in developing countries. Objective: To determine the factors associated with non-adherence to anemia treatment in children aged 6-59 months. Methods: The study is correlational in scope, observational in design, analytical cross-sectional. The sample consisted of 158 caregivers-children. Structured interviews were conducted with validity and reliability obtained by the midpoint distance method, and Cronbach's alpha was calculated to assess reliability. Descriptive statistics were used, the χ² test, crude OR and multiple Poisson regression with robust variance ORa, with a statistical significance value (p<0.05). Results: Non-adherence was 67%. Caregivers who are grandparents or others other than parents (ORa = 4.23, p = 0.018), not having knowledge of anemia and its repercussions on the child's health (ORa = 5.12, p = 0.019), poor caregiver-health staff relationship (ORa = 4.33, p = 0.005), non-availability of the drug (ORa = 6.89, p = 0.012), the use of ferrous sulfate (ORa = 3.55, p < 0.001), presenting adverse effects (ORa = 8.49, p < 0.001), lower-middle, working-class and marginal strata (ORa = 1.93, p = 0.023), were significantly associated with non-adherence. Conclusion: Non-adherence is a multifactorial problem that includes variables related to the child-caregiver, the health care team, the treatment, and the socioeconomic context.
