To determine if hypoproteinemia and hypoalbuminemia are associated with mortality in patients with severe dengue at Hospital de La Amistad II-2 Santa Rosa and Hospital III José Cayetano Heredia, Piura, Peru. Materials and Methods: This case-control study was conducted at Hospital III José Cayetano Heredia and Hospital de La Amistad II-2 Santa Rosa in Piura, Peru. It included 180 patients diagnosed with severe dengue, divided into two groups: 45 mortality cases and 135 survivor controls. Clinical and laboratory data were collected, focusing on hypoproteinemia and hypoalbuminemia levels at hospital admission. Information was obtained from medical records and laboratory analyses. Statistical tests were used to compare the prevalence of hypoproteinemia and hypoalbuminemia between the groups and to assess their association with mortality, contrasting our findings with previous studies for comparative context. Results: Among deceased patients, 77.78% had hypoproteinemia and 80.00% had hypoalbuminemia, whereas in survivors, the proportions were 48.89% and 58.52%, respectively. In terms of comorbidities, 40.00% of deceased patients had hypertension, compared to 15.56% of survivors. Type II diabetes mellitus affected 13.33% of deceased patients and 15.56% of survivors. Cirrhosis and renal disease were present in 6.67% and 13.33% of deceased patients, respectively, versus 2.96% and 2.96% of survivors. Myocarditis affected 6.67% of deceased patients and 4.44% of survivors. Regarding severe complications, 42.22% of deceased patients experienced shock and 33.33% had hemorrhage, compared to 24.44% and 25.93% of survivors, respectively. Conclusions: We found that hypoproteinemia and hypoalbuminemia were significantly more prevalent among deceased patients compared to survivors. Additionally, advanced age, hypertension, and renal insufficiency were significantly associated with higher mortality. These findings highlight the importance of hypoproteinemia and hypoalbuminemia in assessing risk and prognosis in severe dengue
