To know if the night surgery shift is a risk factor for the appearance of postoperative complications in acute appendicitis. Materials and methods: Retrospective cohort study, in which all patients over 18 years of age, of both genders, undergoing appendectomy during the period 2020 - 2022 at the Regional Teaching Hospital of Trujillo participated. Results: A higher incidence of postoperative complications was observed during the night shift of 64.1% (157). The RR was calculated as 1.62 (95% CI 1.35 – 1.94) (p <0.001), demonstrating with this result that patients undergoing appendectomy during the night shift are 1.62 times more likely to present postoperative complications than those who did not undergo surgery at night. On the other hand, it is evident that the intervening variables; Age, disease duration, time to surgery, ASA score, leukocytosis, surgical technique, surgery time, primary surgeon and operative findings have a statistically significant association, with (p 0.001), (p <0.001), (p <0.001), (p <0.001), (p <0.001), (p <0.001), (p 0.0032), (p <0.001), (p <0.001) and (p <0.001), respectively. Conclusions: The night shift of surgery is a risk factor for the appearance of postoperative complications.
