Abstract: Objective: To determine if the immunity index systemic inflammation has
a predictive value of sepsis.
Material and methods: A study of diagnostic tests was carried out in
which 198 adult patients with suspected sepsis were included, according
to selection criteria which were divided into 2 groups: with sepsis and
without sepsis; the ROC curve and the Youden index were calculated to
find the best cutoff point for the diagnosis of sepsis. Based on the above,
sensitivy, specificity, positive and negative predictive value was
calculated.
Results: The sensitivity and specificity of the immunity index systemic
inflammation as a predictor of sepsis was 61% and 84% respectively. The
positive predictive value and negative predictive value of immunity
systemic inflammation as a predictor of sepsis was 53% and 88%
respectively. The prognostic accuracy of the systemic inflammation
immunity index as a predictor of sepsis was 73%, corresponding to an
intermediate accuracy. No significant differences were observed
regarding the variables age, gender, diabetes mellitus, hypertension or
ischemic heart disease among patients with or without sepsis.
Conclusion: The immunity index systemic inflammation has a predictive
value of sepsis.
Objetivo: Determinar si el índice de inmunidad inflamación sistémica tiene valor predictor de sepsis. Material y métodos: Se llevó a cabo un estudio de pruebas diagnósticas en el que se incluyeron a 198 pacientes adultos con sospecha de sepsis, según criterios de selección los cuales se dividieron en 2 grupos: Con sepsis y sin sepsis. Se calculó la curva ROC y el índice de Youden para encontrar el mejor punto de corte de Índice de Inmunidad-Inflamación sistémica para diagnóstico de sepsis, a partir de lo anterior se calculó la sensibilidad, especificidad, el valor predictivo positivo y valor predictivo negativo. Resultados: La sensibilidad y especificidad del índice de inmunidad inflamación sistémica como predictor de sepsis fue de 61% y 84% respectivamente. El valor predictivo positivo y el valor predictivo negativo de inmunidad inflamación sistémica como predictor de sepsis fue de 53% y 88%, respectivamente. La exactitud pronóstica del índice de inmunidad inflamación sistémica como predictor de sepsis fue de 70% correspondiendo a una exactitud intermedia. No se apreciaron diferencias significativas respecto a las variables edad, género, diabetes mellitus, hipertensión arterial ni cardiopatía isquémica entre los pacientes con o sin sepsis. Conclusión: El índice de inmunidad inflamación sistémica tiene valor predictor de sepsis.
