Abstract: Antecedentes: Las complicaciones de la diabetes mellitus se pueden evitar si la enfermedad se detecta, se trata y se mantiene bajo control. Pero, algunos estudios previos muestran que en Sudamérica los niveles de tratamiento y control son inadecuados. Objetivo: Determinar la proporción de pacientes adultos con DMT2 que son concientes de su enfermedad, que reciben tratamiento y que están controlados en Sudamérica. Métodos: La búsqueda se realizó en Medline, SCOPUS y LILACS, en los últimos 5 años. Se incluyeron estudios observacionales de países sudamericanos publicados en español, inglés o portugués; que estudien la proporción de conciencia, tratamiento y control de personas con diabetes. Se realizó una síntesis cualitativa con los resultados principales, y un metanálisis de cada desenlace mediante el modelo de efectos aleatorios. Además, se evaluó el riesgo de sesgo Resultados: 14 artículos cumplieron con criterios de inclusión. El metanálisis de conciencia de enfermedad incluyó a 5 estudios y se encontró un estimado final de 72,2% (IC95%:64,0%-80,0%, I2:93,1%). El metanálisis de tratamiento de enfermedad provino de 3 estudios y se encontró un estimado final de 66,3% (IC95%:52,6%-78,8%, I2:98,6%). El metanálisis de control de enfermedad provino de 5 estudios y se encontró un estimado final de 43,8% (IC95%:38,6%-49,0%, I2:95,0%). Todos nuestros estudios incluidos tuvieron un riesgo de sesgo bajo. Conclusiones: En Sudamérica, es alta la proporción de personas con diabetes que no son conscientes de su enfermedad, no reciben tratamiento ni están controlados. Deben implementarse medidas para mejorar el acceso a diagnóstico, tratamiento y control de la diabetes.

Background: Complications of diabetes mellitus can be avoided if the disease remains under control. But, previous studies in South America showed that the treatment and control of diabetes are inadequate. Objective: Determine the proportion of patients with diabetes that are aware of their disease, treated and controlled in South America. Methods: We conducted a systematic review with meta-analysis. Our search was carried out in Medline, SCOPUS and LILACS, limited to the last 5 years. We included observational studies from South American countries published in Spanish, English and Portuguese; those that included the proportion of awareness, treatment and control of participants with diabetes. A qualitative synthesis was performed with the main results, and a meta-analysis of each outcome using the random effects model. Additionally, the risk of bias was assessed. Results: 14 studies met the inclusion criteria. The disease awareness meta-analysis included 5 studies and found an estimate of 72,2% (IC95%: 64,0%-80,0%, I2: 93,1%). The disease treatment meta-analysis included 3 studies and found an estimate of 66,3% (IC95%: 52,6%-78,8%, I2:98,6%). The disease control meta-analysis included 5 studies and found an estimate of 43,8% (IC95%: 38,6%-49,0%, I2:95,0%). All of our studies were at low risk of bias. Conclusions: In South America, the proportion of people with diabetes that are not aware of their disease, not treated and not controlled are high. Measures to improve diabetes diagnosis, treatment and control should be implemented.
