Chronic kidney failure (CRF), a highly prevalent disease, hospital morbidity and mortality, the evaluation of the use of drugs in CRF was carried out through Pharmacotherapeutic Follow-up (SFT) and serum creatinine in patients hospitalized in Internal Medicine of Hospital III-Iquitos EsSalud, from May to December 2019. This research was carried out with 54 hospitalized patients of both sexes, aged 45 to over 65 years. The methodology is quantitative, with a quasi-experimental design because blood creatinine levels were measured before and after CRF patients receiving pharmacological treatment, descriptive because a pharmaceutical intervention was carried out through pharmacotherapeutic follow-up and longitudinal because it was performed in the period from May to December 2019. The results obtained, male sex 59.3%, from 56 to over 65 years 46.3%, incidence of PRM 90.7%, Effectiveness category 98.1% and the highest incidence was PRM1 with 83.3% for not receiving the medications you need in a timely manner. The hospital stay was 68.5% (less than 7 days). Student's t test 0.024 (p <0.050), the proposed hypothesis is verified, that the SFT is related to serum creatinine values, decreased hospital stay, improved quality of life, decreased hospital costs and decreased comorbidity associated with other pathologies. Conclusion: "Pharmacotherapeutic follow-up in patients with chronic renal failure is associated with the proper use of medications and a decrease in serum creatinine levels in hospitalized patients at Hospital III Iquitos EsSalud 2019".
