Introduction: The wounds that are produced by firearm projectile are currently an emerging public health and social problem worldwide. The fractures related to the injury, produced by firearms, are currently little studied and constitute a very serious challenge for the doctor who deals with this type of injury, so the present study makes this type of lesions visible locally. Objectives: To determine the clinical characteristics and the management of fractures caused by firearm projectile in patients treated at the "César Garayar García" Hospital, between the years 2016-2018. Methodology: A non-experimental, cross-sectional study was conducted in all patients admitted with the diagnosis of a firearm fracture to the Hospital "César Garayar García" in the years 2016-2018. Results: Between the years 2016 to 2018, 33 patients were studied, with an average age of 34 years, 32 male patients (97%) and only 1 female patient. Of all the patients, 42.4% (14) came from rural communities or districts far from the Hospital; 51% managed to reach the hospital within 4 hours of the injury, with all the injuries classified in the Gustillo classification as Grade III (III-A with 69.69%, III-B and III-C, with the 15.15% respectively. Valuing the injured bones, the most affected were the hands (36.4%), followed by the bones of the foot (21.2%). 91% (30 patients) were treated conservatively; the antibiotic treatment of choice in the majority of cases (20 patients) was the combination of cefazolin 500 mg c / 6h + gentamicin160 mg c / 24h E.V; the mortality was 9.1% (3 patients). The presence of other lesions (such as chest injury), risk for life, and fractures classified according to Gustillo in III-B and III-C, was associated with higher mortality. Conclusions: The patients who presented fractures by firearm more affected were the males, especially the minors of 30 years. Of the patients with firearm fractures, 40% came from rural communities where hunting activities are carried out. The average time of arrival at the Hospital was 4 hours; Mortality of patients injured by firearm was 9%.
