Introduction: The medical risks associated with pregnancy in adolescent mothers, such as hypertensive disease, anemia, low birth weight, premature birth, among others, determine an increase in maternal morbi-mortality and an estimated increase of 2 to 3 times in the infant mortality, when compared to the most advanced maternal age groups. At the national level, these data are scarce and are not properly updated. Objectives: To determine the relationship between adolescent pregnancy and maternal and perinatal complications at the Cesar Garayar García Iquitos Hospital, January-December 2017. Material and Method: A retrospective study of cases and controls was carried out to determine the relationship between adolescent pregnancy and maternal and perinatal complications. It was found 828 pregnant adolescents who delivered at the César Garayar García Iquitos Hospital in 2017, a sample of 263 cases and 263 controls was obtained. The sampling was probabilistic. It was use chi square, calculating the OR indicating the 95% confidence interval. A p<0.05 was considered significant. Results: We found a higher probability of maternal and perinatal complications in adolescent pregnant women with a non-deficient educational level, unpaid, with a stable civil union, of rural origin, without harmful habits and without a pathological background. As well as in first-pregnant adolescents, with no antecedent of abortion, with inadequate prenatal control and who had cesarean delivery. The 87% children of teenage mothers had an adequate average weight. The average value of height, head circumference and chest circumference were 49.48cm, 33.31cm and 32.81cm, respectively. The average values of Apgar 1´/5´ were 8,27/9,49; and Age per Capurro method of 38.03. It was found that the probability of complication is 4 times higher in pregnant adolescents, being statistically significant: Anemia OR:3.8(2.5-5.8), Pre-eclampsia OR:2.3(1.2-4.6), Preterm birth OR: 3.5(1.5-7.8) and precipitated birth OR:2.1(1.1-4.2). It was found that the probability of having perinatal complications among pregnant adolescents is 3 times higher than that of pregnant adults. The complications being statistically significant: Prematurity OR:2.9(1.5-5.5), neonatal sepsis OR:3.3(1.2-9.2), low birth weight OR:3.1(1.5-6.2) and small for gestational age OR:2.4(1.5-3.7). Conclusions: Adolescent pregnancy has a statistically significant relationship with maternal and perinatal complications, being the probability of maternal complication 2 times higher than perinatal complications
