Análisis del costo de referencia de pacientes de la red asistencial de Essalud Loreto a la ciudad de Lima, durante el periodo 2011 - 2015

Abstract

La Red Asistencia de EsSalud Loreto no cuenta con el suficiente capital humano, equipos de diagnóstico y tratamiento, e infraestructura, para satisfacer la demanda de los pacientes que llegan buscando recuperar la salud. Por este motivo, se tiene que referenciar (trasladar) a los pacientes a la ciudad de Lima, para que sean atendidos en hospitales nacionales como el Hospital Rebaglaiti, Hospital Almenara, entre otros, los que cuentan con todo lo necesario para la atención, pero se encuentran saturados. El costo de referenciar un paciente es muy elevado, siendo el costo del pasaje aéreo el 50% del costo total, ya que no solamente coberturan el costo de traslado del paciente sino también, en algunos casos, el costo del acompañante. El objetivo de la presente tesis magistral fue analizar el costo de referencia de los pacientes de la Red Asistencial de EsSalud Loreto a la ciudad de Lima, durante el Periodo 2011 – 2015. Para ello, se llevó a cabo la presente investigación de tipo Descriptivo y de diseño No Experimental, analizando el comportamiento de los costos de referencia, conformado por los “Costos Por Pasaje” y “Otros Costos”, así como el número de pacientes referenciados, los motivos más frecuentes de la referencia y el costo asociado al motivo de la referencia. En este punto, el motivo más frecuente por el que se referenciaron pacientes a Lima fue “Consulta Externa” con el 92.39% del total de pacientes. Cuando se analizó el costo asociado al motivo de la referencia, se pudo comprobar que el mayor costo fue por “Consulta Externa”, el mismo que representó el 92.12% del costo total. Finalmente, se compararon los costos necesarios para dotar a la Red Asistencia de EsSalud Loreto de condiciones mínimas para dejar de referenciar pacientes, tales como equipos de tratamiento y diagnóstico, recursos humanos (médicos, enfermeras, técnicos, tecnólogos, administrativos, etc.), infraestructura, materiales médicos e insumos; en relación a los costos de referenciar pacientes a la ciudad de Lima; es decir, entre tratar a un paciente en la ciudad de Iquitos con todo lo necesario o enviarle a la Lima, concluyendo que referenciar a un paciente a Lima durante el periodo 2011 – 2015 ocasionó un gasto mayor de S/. 9,821,621.32; por lo que, de lejos, la alternativa de equipar la Red Asistencial de EsSalud Loreto es la más adecuada.

The EsSalud Loreto Assistance Network does not have enough human capital, diagnostic and treatment equipment, and infrastructure, to meet the demand of patients who arrive to recover their health. Finally, the necessary costs were compared to provide the EsSalud Loreto Assistance Network with minimum conditions to stop referencing patients, such as treatment and diagnosis teams, human resources (doctors, nurses, technicians, technologists, administrative staff, etc.) infrastructure, medical materials and supplies; in relation to the costs of referring patients to the city of Lima; that is, between treating a patient in the city of Iquitos with everything necessary or sending him to Lima, concluding that referencing a patient to Lima during the period 2011 - 2015 caused a higher expenditure of S /. 9,821,621.32; so that, by far, the alternative to equip the EsSalud Loreto Care Network is the most appropriate.
