The objective of this research was to determine the degree of surgical complexity in lower third molars according to the Pederson and Zhang index in Cone Beam Tomographies of the Luis Vallejos Santoni University Stomatological Center of the Andean University of Cusco, 2023. For this, the methodology used had a quantitative approach, it was descriptive in scope and non-experimental, retrospective observational and documentary design; composed of a population of 1175 Cone Beam Computed Tomographies and a sample of 290. As results of the investigation for the degree of surgical difficulty according to Pederson it was "Moderately Difficult" (77.9%); followed by "Little Difficult" (13.8%) and "Very Difficult" (8.3%); for the degree of surgical difficulty according to Zhang it was "Little Difficult" (78.6%), followed by "Moderately Difficult" (20.7%) and "Difficult" (0.7%). Concluding that in the degree of surgical difficulty in lower third molars for the Pederson index it was "Moderately Difficult" while for the Zhang index it was "Little difficult". Demonstrating that it is very useful to assess these indices according to the different criteria presented by their categories before the pre- surgical act.
