Background: According to the “burden of stroke, 2019“, in the world more than 11 million people are affected by an ischemic stroke each year, more than half of this figure occurs in people from low- and middle-income countries like ours. The incidence of ischemic stroke increases with age and approximately 10 to 20% of these events occur in people aged 15 to 45 years, this disorder being the second cause of death worldwide, and the second cause of disability according to the World Organization Of the health. In 2020, in a systematic review carried out by Putaala J., et al., they found that the main factors for the development of ischemic stroke vary in frequency from the classic ones reported for people over 60 years of age, these being in order of frequency: Smoking, Sedentary lifestyle, Atrial Fibrillation, Arterial Hypertension, Dyslipidemia, and much less frequently: Diabetes Mellitus, Migraine, Alcoholism, and Obstructive Apnea. Methods: This is a retrospective, observational, analytical case-control study. It was carried out through a data collection form, validated by experts and applied to 202 patients from the Hospitals: Adolfo Guevara Velasco and Antonio Lorena from the city of Cusco,of which 101 were cases and 101 controls with a 1:1 ratio. For statistical analysis, the odds test and Pearson's chi-square test were used. Results: From the univariate analysis, it was obtained that the highest frequency occurs in the female sex (58.4%) and in the age group between 30 and 45 years, with a mean presentation at 32 years. the most frequently found etiology was undetermined with 34.7%, followed by cardioembolism with 27.7%. In the bivariate analysis, we obtained as risk factors: Arterial Hypertension (OR=2.99 CI=95%, p=0.005), Dyslipidemia (OR=2.13, CI=95%, p=0.027), Use of Oral contraceptives n=118, (OR=3.05, CI=95%, p=0.041), Smoking (OR=3.82, CI=95%, p=0.012), Previous Stroke (OR=3.08 , CI=95%, p=0.03), Atrial Fibrillation (OR=3.52, CI=95%, p=0.004) and COVID19 (OR=2.38, CI=95%, p=0.031). Non-associated risk factors were Diabetes Mellitus: (OR=1.1, CI=95%, p=0.82), Alcoholism (OR=1.54, CI=95%, p=0.42), Migraine (OR=1.60, CI=95%, p=0.23), Obesity (OR=1.50, CI=95%, p=0.31). Conclusions: Ischemic stroke affects young people between 15 and 45 years of age with a higher prevalence in women than in men, the most frequent etiology in our setting is undetermined. The most important risk factors for the development of stroke in this age group are in order of importance: Smoking, Atrial Fibrillation, Previous Stroke, Use of Oral Contraceptives, Arterial Hypertension, COVID-19 and dyslipidemia.
