Introduction: Acute cholecystitis continues to be a common problem, with a mortality rate of 0.6%. The Tokyo 2018 severity criteria for this pathology are categorized into 3 grades (I, II, and III) and help not only in decision-making regarding management, but also in predicting complications (organic complications, operative time, hospital stay, surgical transformation, post- surgical admission). Objective: To demonstrate the effectiveness of the Tokyo severity criteria as a predictor of complications in post-cholecystectomy patients at the Antonio Lorena Hospital in Cusco, 2023. Methods: An observational, quantitative, descriptive, cross-sectional, retrospective, analytical study was carried out using ROC and AUC curves, as well as a confusion matrix to determine sensitivity and specificity to determine prognostic efficacy. Results: Of a total of 114 clinical records reviewed, 57.01% (65) were grade I, 41.22% (47) grade II and 1.75% (2) grade III; Regarding the operative time, it has been determined that the CST is a poor predictor (AUC=0.691, CI (0.608-0.775)). For the hospital stay, the CST (Tokyo Severity Criteria) is an excellent predictor (AUC=0.923, CI = (0.874-0.973)). Finally, for surgical transformation, the CST is a moderate predictor (AUC=0.772, CI = 0.678-0.866)). Conclusion: The Tokyo 2018 Severity Criteria for acute cholecystitis have an effective prognostic value for hospital stay length, surgical transformation, and operative time in post-cholecystectomy patients at HAL 2023.
