INTRODUCTION: COVID-19, identified in 2020, poses a global challenge to public health, being a potentially deadly systemic disease that primarily affects the lungs but can also cause liver damage. This damage can result from the direct action of the virus, hyperactive immune response, or toxicity of certain medications used in its treatment. The severity of liver damage varies with the severity of the infection, being crucial for patient prognosis and survival. Early detection of liver abnormalities may indicate disease severity and has been observed in a high percentage of hospitalized COVID-19 patients, highlighting the importance of studying and managing these complications effectively. OBJECTIVE: To determine the factors associated with liver damage in patients hospitalized for COVID-19 at Antonio Lorena Hospital in Cusco, 2020 - 2022. MATERIALS AND METHODS: The study conducted at Antonio Lorena Hospital in Cusco between 2020 and 2022 aimed to identify the relationship between risk factors for liver damage in COVID-19 patients. An analytical cross-sectional design was employed to assess variables of interest and determine the prevalence and characteristics of liver damage, as well as associated factors. The study population comprised patients hospitalized for COVID-19 during this period, with specific inclusion and exclusion criteria. The sample size was calculated based on a previous study and selected using probabilistic-random sampling. Data collection was performed through documentary review of medical records, using a data collection form validated by experts. Data analysis included univariate and bivariate analyses to establish associations between variables, using measures such as Odds Ratios and p-values to assess statistical significance. SPSS 27.0 software was used for statistical analysis. RESULTS: From the analysis of 198 medical records of patients hospitalized at Antonio Lorena Hospital in Cusco due to COVID-19, it was found that 25.3% had liver damage, distributed equally between men and women. Patients aged 31 to 50 years showed the highest percentage of liver damage (34%), while those aged 18 to 30 years had the lowest percentage (10%). Regarding liver profile alterations as markers of liver damage, it was observed that 60% of affected patients had alterations in a single marker, followed by 22% with alterations in two markers, 14% in three markers, and 4% in four markers. Treatment of COVID-19 with corticosteroids showed a significantly higher probability of presenting alterations in ALT, AST, and albumin levels, as well as a higher risk of developing liver damage compared to those who did not receive this treatment. The severity of COVID-19 was identified as a risk factor for liver damage. In severe cases, characterized by oxygen saturation below 90%, a significantly higher probability of presenting hepatic value alterations was observed, with a higher risk of developing liver damage compared to less severe cases. CONCLUSIONS: In our study at Antonio Lorena Hospital, we identified a significant relationship between liver damage and several factors related to COVID-19 treatment and severity. We found that 25.3% of patients developed liver damage, with a similar incidence between men and women. Additionally, we observed that age also influences, with the 31 to 50 age group being the most affected. Regarding liver profile alterations, we found that the majority of patients with liver damage had alterations in one or two markers of the liver profile. Treatment with corticosteroids significantly increased the risk of liver damage, as did the severity of COVID-19 infection, especially in cases with low oxygen saturation. It is crucial to consider these liver abnormalities in COVID-19 patients and the factors identified in our study to prevent potential liver complications.
