Background: Background: Preterm birth is one of the main causes of perinatal morbidity and mortality in the world. It is recognized that its origin is multifactorial, having in greater prevalence the physiological and biological aspects. Studies and strategies were carried out worldwide to reduce its incidence, but in the last 10 years the percentage of preterm births has not decreased, and that is why other causes are sought to explain it. Thus psychopathological factors such as stress have emerged as an important risk factor. In Peru, 6.8% of preterm births are responsible for 29% of perinatal deaths. The aim of the study was to determine the relationship between stress and preterm delivery in pregnant women between 20 and 40 years in the Antonio Lorena Hospital and Regional Hospital of Cusco. Methods: A case-control study was conducted at Antonio Lorena Hospital and Regional Hospital of Cusco, May 2022 - February 2023. 130 cases and 260 controls were identified, considering any pregnant woman less than 37 weeks of gestation with diagnosis of preterm birth and control to all pregnant >= to 37 weeks and with labor at term. Univariate statistical analysis was used to find frequencies and percentages, bivariate analysis to find the OR with 95% CI and p value, and multivariate analysis with logistic regression. Results: It was found in this study that pregnant women with risk of stress presented more preterm birth compared to those without stress (68.5% vs 31.5%) and that stress was associated with a higher probability of preterm labor with an ORc=4,548 with HF [2.8947.148], p=0.000, no relationship of stress and preterm birth with general characteristics was found. Conclusions: It was found that there is a significant relationship between stress and spontaneous preterm birth.
