Introduction: Prematurity is the first cause of mortality in children less than five years old. In Peru in 2015, 9 out of 10 women completed their pregnancy with a gestational age of 37 weeks or more, evidencing 6.5% (27 thousand) of newborns with less than 37 weeks, generating not only risk of mortality and morbidity if not a social and family economic cost. Objective: To determine if the predominant maternal risk factors associated with preterm delivery vaginally in the Regional Hospital Guillermo Díaz de la Vega Abancay2016. Method: Analytical, retrospective study of cases and controls, Population 936 vaginal deliveries, with non-probabilistic sampling, 130 cases and 130 controls were obtained. The clinical histories were reviewed, applying the inclusion and exclusion criteria, as well as the data collection form and preparation of a data sheet in SPSS version 23, for the descriptive analysis with level of significance p <0.05. It was considered as a risk factor if OR> 1 and 95% CI. Results: Maternal age was between 20 and 35 years old. The obstetric factors were; antecedent of preterm birth (OR: 1.071, 95% CI, 1.099-1.540), nulliparity (OR: 1.101, 95% CI 1.123-1334), short intergenic period (OR: 1.330, 95% CI 1.406-2.196), inadequate prenatal control ( OR: 1293, 95% CI 1.512-2.461), overweight and obesity (OR = 1143, 95% CI, 1.038 -2.302) presence of urinary tract infections (OR: 1.082, 95% CI 1.099-1.78), and presence of premature rupture of membranes (OR: 1144; 95% CI: 1,845-4,238). Other factors, low educational level (OR: 1488, 95% CI 2.245-9.057), anemia and pregnancyinduced hypertension (OR: 1353, 95% CI 1.805-2.356). It was also observed that 86.9% of preterm births were late preterm births. Conclusion: The predominant maternal risk factors associated with preterm birth at the Guillermo Díaz de la Vega Regional Hospital were pregnant women who had a history of preterm birth, nulliparity, short intergenic period and inadequate prenatal control, urinary tract infection, premature rupture of membranes, low educational level, overweight and pathologies during pregnancy such as anemia and hypertension induced by pregnancy
