Introduction: Patients with concomitant conditions, such as obesity, and the elderly were more likely to have severe cases during the COVID-19 pandemic because of several mechanical and inflammatory factors, since people with obesity have a compromised immune system, which, when combined with chronic inflammation, makes it difficult for respiratory muscles to respond to low oxygen levels. Objective: To determine if there is an association between obesity and the development of severity and mortality in patients infected with COVID-19 in the Regional Hospital of Cusco in the year 2021. Materials and methods: The research was cross-sectional, retrospective, observational and correlational. Information was obtained from the medical records of the study population, which consisted of patients aged 18 years or older belonging to the Hospital Regional del Cusco attended during the year 2021 and who were diagnosed with obesity or who had a BMI greater than or equal to 30 kg/m2 or more, who also had COVID-19 infection validated by biochemical or molecular tests. Results: A total of 4557 clinical records were examined, of which 167 met the established selectivity criteria and were subjected to analysis. More than 50% of the patients in the study were women (52.7%), while the proportion of men was lower and was represented by 47.3%, which shows a female predominance in the study. According to age, a prevalence of the population ≤ 43 years was noted with 35.3%. Of the patients, 27.5% presented clinical severity and less than half (32.3%) died. Furthermore, a statistically significant association of obesity class related to severity (p=0.00, p<0.05) and mortality (p=0.00, p<0.05) was evidenced. Conclusions: A statistically significant association has been identified between obesity with the development of severity and mortality in patients with COVID-19 in the Regional Hospital of Cusco in the year 2021, which means that the class of obesity correlates with the presence of severity and mortality.
