Introduction: Gastric cancer is a disease with a high mortality rate, occupying the second place in our country, and it is the deadliest neoplasm in men in Cusco. For this reason, it is necessary to obtain an early assessment of the prognosis of patients. Objective: To determine the correlation between laboratory parameters and prognosis in patients with gastric cancer at the Adolfo Guevara Velazco National Hospital, period 2015-2023. Methods: Observational, correlational, retrospective and cross-sectional study. With a total sample of 77 clinical histories of the Adolfo Guevara Velazco National Hospital with the diagnosis of gastric adenocarcinoma between the years 2015 and 2023. The frequencies, percentages and measures of central tendency were obtained, for the correlation of albumin and neutrophil-lymphocyte index with the clinical staging was used the Student's T and U Mann Whitney hypothesis tests. Results: the mean age was 65.29 ± 13.27. the predominant sex was male with 82%. 67% of patients were found in an advanced clinical stage (III-IV) at the time of diagnosis. In the bivariate analysis, a significant association of the neutrophil-lymphocyte index with advanced clinical stage was found (OR: 8.65 95% CI 2.73 – 27.44 p<0.001), similarly with low serum albumin values (OR: 1.59 95% CI 0.482-5.262 p<0.029) Conclusions: the neutrophil-lymphocyte index is independent predictor of advanced clinical stage in gastric cancer.
