The objective of the research work is to determine the relationship between caries, loss of permanent first molars and the oral hygiene index in schoolchildren from 6 to 12 years old of the I.E. Bolivarian Cusco 2018. This descriptive-correlational type research was carried out, using observational technique and quantitative scope. The sample consisted of a total of 195 students of the Bolivarian Educational Institution, from first to sixth grade of primary school. For the evaluation, a clinical oral examination was performed following the parameters established by the instrument, where the following were considered: the clune index and the simplified oral hygiene index. For the analysis of the results, the chi-square distribution (χ²) was used; the statistical test of kruskal-walli, correlation coefficient of spearman and U of Mann-Whitney. The results showed that there is a significant relationship between caries, loss of permanent first molars and oral hygiene index in the Bolivarian Educational Institution. Where the level of caries was slight with 46.2%, with a predominance in the female sex and the most affected age was at 12 years with 9.2%. It was determined that the loss of the first permanent molar was low with 12.8%, with a predominance in the male sex, and the most affected age was at 11 years, likewise it was identified that the most affected parts were the first lower left molar (3.6) and the first lower right molar (4.6) with 4.6%. The oral hygiene index was regular 70.3% in all groups. Finally, it is concluded that if there is a highly significant relationship between caries, loss and oral hygiene index
