To determine whether intraoperative diagnostic accuracy compared to histopathological diagnosis affects postoperative treatment in patients with acute appendicitis. Methods: A cross-sectional study was conducted at the Regional Teaching Hospital of Trujillo between January 2019 and December 2021. A sample size of 290 patients operated on for acute appendicitis was calculated. Non-probabilistic sampling was used. Sociodemographic, clinical, operative, and histopathological data were collected from medical records. Descriptive statistical analysis was performed, and association and correlation tests were applied. Results: Of the 290 patients analyzed, a balanced distribution by sex and an average age of 32.24 ± 11.53 years were observed. Significant differences were found between the classifications of surgeons and pathologists in the stages of acute appendicitis (p < 0.001). Additionally, statistically significant correlations were identified between macroscopic and microscopic classifications of samples and leukocyte count (p < 0.001). The duration of antibiotic treatment varied according to the histological stage of appendicitis, with a longer median in cases of perforated appendicitis. Conclusion: The results suggest that intraoperative diagnostic accuracy may influence postoperative treatment in patients with acute appendicitis. The discrepancy between the classifications of surgeons and pathologists highlights the importance of accurate communication between these professionals.
