Objective: To determine whether inflammatory parameters are predictive factors of complicated acute appendicitis in patients with appendectomy at the Belén Hospital in Trujillo from 2019 to 2022. Material and methods: Cross-sectional, observational and analytical study. The study sample consisted of 165 patients, who were divided into two groups, the first group with acute complicated appendicitis made up of 109 patients and the second group with uncomplicated acute appendicitis made up of 56 patients. Data analysis was performed using the Chi-square test, in addition to the calculation of the Odds Ratio (OR), considering a significance of 5% and a 95% confidence interval. Results: It was found that leukocytosis (p=0.048, OR=1.26, OC 95%), increased neutrophil/lymphocyte ratio (p=0.010, OR=1.34, OC 95%), elevated C-Reactive protein (p˂0.001, OR=1.77, OC 95%); the lymphocyte/monocyte ratio (p=0.001, OR=0.64, OC 95%) and were the inflammatory parameters predictive of complicated acute appendicitis. Conclusion: Elevated CRP and increased neutrophil/lymphocyte ratio were inflammatory parameters predictive of complicated acute appendicitis in patients with appendectomy at the Belén Hospital in Trujillo from 2019 to 2022
