To determine if asymptomatic bacteriuria is a risk factor for bacterial resistance in women over 40 years old within the gynecology service at EsSalud Piura hospital. MATERIAL AND METHOD: This is a quantitative, observational, analytical case-control study. The sample size consisted of 99 patients, all of whom were distributed into two groups based on whether they had a diagnosis of asymptomatic bacteriuria or not. The analysis will be bivariate, testing the hypothesis using the Chi-square test or Fisher's exact test, considering the test assumptions, with p-values < 0.05 taken as significant. RESULTS: It was found that the average age was similar between both groups: 54.27 years for those with bacteriuria and 55.08 years for those without bacteriuria. Women with bacteriuria showed a lower prevalence of being married (31.31%) and housewives (29.29%) compared to those without bacteriuria (50.51% and 47.47%, respectively). Regarding comorbidities, type 2 diabetes mellitus and previous urinary infections were significantly more common in the group with asymptomatic bacteriuria, with OR of 1.01 and 2.12, respectively. There were no significant differences in alcohol and tobacco consumption between the groups. Although antibiotic resistance was higher in the group without asymptomatic bacteriuria (20.20% vs. 15.15%), this difference did not reach statistical significance (p = 0.076). These results suggest a possible association between asymptomatic bacteriuria and certain conditions, but no significant difference in bacterial resistance was found. CONCLUSION: Asymptomatic bacteriuria is NOT a risk factor for bacterial resistance in women over 40 years old within the gynecology service at EsSalud Piura hospital.
