Introduction: In the pediatric population, burns are one of the leading causes of morbidity and mortality, with second- and third-degree burns most frequently requiring admission to the intensive care unit (ICU). The lactate/albumin ratio (LAR) has been found to have good prognostic performance for mortality in adult burn patients; however, it has not been studied in the pediatric population. Objective: To evaluate the prognostic performance of the lactate/albumin ratio for complications in children aged 5 years or younger with second- and/or third-degree burns in the intensive care unit. Methods: A retrospective cohort study of prognostic tests was conducted, analyzing the medical records of children aged 5 years or younger with second- and/or third-degree burns admitted to the ICU in a hospital in Lima, Peru, during the period from 2017 to 2022. The lactate/albumin ratio was calculated from lactate and albumin values taken within 48 hours of the injury. The area under the curve (AUC) was calculated to determine the prognostic performance of the LAR for complications. Results: A total of 258 patients aged 5 years or younger with second- and/or third-degree burns were studied. Among these patients, 63 experienced complications (24.4%). An AUC of 0.63 (95% CI: 0.54-0.71) was estimated for the LAR. An elevated LAR was associated with an increased risk of complications (aRR: 2.23; 95% CI: 1.51-3.29; p<0.001) after adjusting for confounding variables. Conclusion: The LAR did not show good prognostic performance for complications in pediatric burn patients; however, a significant association was found between an elevated LAR and an increased risk of developing complications.
