INTRODUCTION: Sepsis is one of the most common fatal diseases. It is one of the few medical conditions that attacks both populations in low-income areas and those in the high-income world equally and with great ferocity. Sepsis continues to be one of the main causes of death in critically ill patients in emergency services, between 20 to 50% of severely affected patients die, in addition to reducing the quality of life of those who survive. The study seeks to compare two tests for the diagnosis of patients with sepsis, as well as their ability to predict mortality; the two tests chosen are the SIRS criteria, proposed in the first sepsis consensus in 1991; against the qSOFA score, raised in the third consensus of 2016. OBJECTIVE: To determine the diagnostic value and mortality prediction of the qSOFA score compared to the SIRS criteria for sepsis in patients treated with suspected infectious causes in the emergency service of the Hospital Apoyo Iquitos "César Garayar García", from October to December 2017. METHODS: Descriptive Study - Observational: Cross-sectional, Retrospective. The population of the study is constituted by all the patients attended in emergency of the Hospital Apoyo Iquitos "César Garayar García", during the period between October 1st, 2017 and December 31st, 2017. The data collection was through cards, directly from: clinical records of patients with suspected infectious origin treated for more than 24 hours in the emergency service and hospital records of the emergency service. The ROC curve and the area under the ROC curve were analyzed for the SOFA and qSOFA scales; both for diagnosis and as a predictor of mortality. A value of p <0.05 was considered statistically significant. In addition, tests of sensitivity, specificity and predictive values were performed for each test under study, as well as for each criterion. RESULTS: The qSOFA score has a low curve area of 0.636, with p = 0.023, CI = 95% (0.523-0.750), being statistically significant to discriminate patients with sepsis from those who do not; with a sensitivity and low specificity of 64% and 63% respectively, as well as a positive predictive value of 82% and a negative predictive value of 39%. In comparison with the SIRS criteria that present better discrimination of patients with sepsis with a low area of curve of 0.994 with p = 0.000, CI = 95% (0.981-1.000), high sensitivity and specificity of 98% and 100% respectively; with a positive predictive value of 100% and a negative predictive value of 96%. With respect to the prediction of mortality; the qSOFA score presented an area under the curve of 0.712, with p = 0.001, CI = 95% (0.614-0.810); showing a sensitivity and low specificity of 47% and 10% respectively; in comparison with the SIRS criteria they presented an area under the curve of 0.636, with p = 0.028, CI = 95% (0.530-0.742); with a sensitivity and specificity also low of 65% and 6% respectively. CONCLUSIONS: It can be concluded that the qSOFA score has a lower diagnostic value for sepsis in patients with suspected infectious origin treated in the emergency services compared to the SIRS criteria, who presented not only a larger area under the curve; therefore, greater statistical significant relationship; but also with greater sensitivity, specificity and predictive values. In addition, it is concluded that, although both are able to discriminate the patient who will die from those who do not, although with low levels of sensitivity and specificity; the qSOFA criteria showed better values both in the statistically significant relationship, and in the sensitivity and specificity.
