Highly significant associations have been described between the red blood cell distribution width and preeclampsia, even presenting high significance in terms of its association with the degrees of severity. For this reason, this study aims to determine the association between the red cell distribution width and severity of preeclampsia in pregnant women attended at the Iquitos Hospital, 2019. A retrospective analytical study was carried out in 264 patients (134 cases with pre-eclampsia and 134 controls), in whom the red blood cell distribution (RDW) was analyzed. It was observed that there is a statistically significant difference (p <0.001) between the mean RDW of patients with normal pregnancy and preeclampsia (14.23 + 0.56 vs. 15.13 + 0.56). Similarly, it is also observed that there is a statistically significant difference (p = .007) between patients with mild pre-eclampsia and patients with severe preeclampsia (14.82 + 0.94 vs. 15.35 + 1.2). A cutoff point of 14.45 was determined to discriminate patients with preeclampsia (S: 67.9 %, E: 61.2 %, PPV: 63.6 % y PNV: 34.4 %) and a cut-off point of 14.75 to discriminate patients with mild and severe preeclampsia (S: 62 %, E: 54.5 %, PPV: 66.2 % y PNV: 50 %).
