Background: The last stages of chronic liver disease present liver transplantation as the best treatment alternative. The most widely used immunosuppressant in organ transplantation is tacrolimus, which has a narrow therapeutic index and inter- and intraindividual pharmacokinetic variability, making it a suitable candidate for therapeutic monitoring. In Peru, monitoring is not feasible since there are no previous studies that determine the variables that affect tacrolimus pharmacokinetics in post liver transplant patients. Objective: To determine the variables affecting tacrolimus pharmacokinetics in post-liver transplant patients at the Edgardo Rebagliati Martins National Hospital. Methods: A retrospective, observational and analytical study was performed, in which the medical records of the 30 patients over 18 years of age who had undergone liver transplantation in the period 2010-2022 at the Edgardo Rebagliati Martins National Hospital were reviewed. The relationship between tacrolimus serum concentration and results of liver profile, biochemical profile and hemogram taken within the first month after transplantation was established using a logistic regression model designed in the R program. Results: An inversely proportional relationship was determined between tacrolimus serum concentration and transplant time, patient age and leukocyte values; obtaining values of -0.24, -0.05 and -0.13, respectively. A directly proportional relationship was established between tacrolimus serum concentration, TGO values and hematocrit; reaching values of 0.006 and 0.126, respectively. No relationship was determined with variables related to the biochemical profile. Conclusion: The pharmacokinetics of tacrolimus in post liver transplant patients studied is influenced by the time of transplantation, age, leukocyte, TGO and hematocrit values.
