Introduction: Gastroesophageal reflux disease (GERD) has a high prevalence. Many patients persist with symptoms despite antisecretory treatment and are referred for surgery. There is little information on the clinical profile and preoperative studies in patients with GERD in Peru. Objective: To describe clinical, endoscopic characteristics and preoperative functional studies in patients with GERD with surgical management in a private clinic. Materials and methods: Retrospective case series. A descriptive analysis was carried out on a database with patients operated on for GERD between 1996 and 2023 who had endoscopic studies, pHmetry, impedance, manometry and preoperative fluoroscopy. Results: 255 patients were included, 55.7% men with an age and body mass index (BMI) of 47.7 years and 26.7 kg/m2. The duration of illness was 48 months, heartburn occurred in 79.2% and 94.8% received proton pump inhibitors (PPIs). In endoscopy, type I hiatal hernia and Los Angeles (LA) B-C-D esophagitis were the most frequent. Pathological GERD using 24-h pH monitoring was seen in 67.9% but due to impedance in 77.4%. 30.7% presented a motility disorder, with 29.4% having ineffective esophageal motility (IEM). Conclusion: Patients undergoing surgery for GERD are mostly overweight men and heartburn is the most prevalent symptom. Endoscopy showed hiatus hernia and erosive esophagitis in most of the population, and 24-h pHmetry and impedance were pathological. Three out of ten patients present esophageal motility disorders, the most common being IEM. More than half have an incompetent lower esophageal sphincter (LES).
