Introduction: The elderly must perform physical activity to have a healthy old age. The risk of falls can lead to lumbar spondylolisthesis. Guided physical exercise has shown favorable results for delaying the aging process and muscle atrophy, while optimizing stability and balance, improving flexibility, among others benefits. Aim: Describe the physiotherapy approach in a basically active older adult with a diagnosis of lumbar spondylolisthesis, who was treated between September and December 2023 in Lima. Description of professional experience: The case of an elderly patient with lumbar spondylolisthesis that generates intense pain in the affected area with extension to the lower limb that limits his mobility is presented. The physiotherapy intervention had a biopsychosocial approach; the main resource was lumbo-pelvic stabilization exercises accompanied by breathing to activate deep muscles such as the abdominal and lumbar multifidus. Conclusion: The evidence-based physiotherapeutic approach for the elderly with lumbar spondylolisthesis was mainly with lumbo-pelvic stabilization exercises that positively influenced the intensity of pain, optimized their mobility and allowed their readaptation in the physical activity they performed.
