Background: Asymptomatic bacteriuria (ASB) in pregnancy exhibits wide variability in its prevalence, with rates ranging from 1.9% to 86.6%. Escherichia coli is identified as the most common etiological agent, although other gramnegative bacteria such as Klebsiella spp. and Proteus mirabilis, and Group B Streptococcus have also been implicated. Treatment selection is based on antibiotic sensitivity, but increasing resistance complicates choice. Studies show significant regional variations in resistance. General Objective: To determine the prevalence of asymptomatic bacteriuria in pregnant patients and identify the implicated microorganisms and antibiotic sensitivity. Materials and Methods: The descriptive, retrospective, observational, and cross-sectional study was conducted at Clínica Internacional (Lima, Peru) between January 2021 and December 2022. The population included pregnant women attended for vaginal delivery and cesarean section. Results: A total of 1927 pregnant women were studied between 2021 and 2022, finding a prevalence of 7.42% of asymptomatic bacteriuria (ASB). In 2021, it was found in 6.4%, of which 43.75% occurred in the first trimester. In 2022, it was found in 8.52% of cases, of which 58.22% occurred in the first trimester. Escherichia coli was the predominant microorganism (74.83% in total), followed by Streptococcus agalactiae (6.29%) and Proteus mirabilis (2.80%). Resistance to several antibiotics was observed, with Trimethoprim- Sulfamethoxazole being the most common. In 2022, resistance increased, especially in E. coli. Conclusion: ASB in pregnant women requires monitoring to prevent complications and select safe and effective treatments.
