Introduction: the progression of chronic kidney disease (CKD) determines the prognosis of the disease, the lower renal survival means higher costs and more complications, it is essential to identify these patients early to initiate timely management. Objectives: Determine the prognostic factors related to renal progression and survival in patients with CKD stage 3 and 4 and the frequency of deaths and associated characteristics in patients with CKD stage 3 and 4 at the Edgardo Rebagliati Martins National Hospital (HNERM). Material and methods: retrospective longitudinal cohort study of patients who entered the HNERM from 2013 to 2019. A multivariate logistic regression analysis was performed to evaluate the factors associated with the progression of CKD, Kaplan- Meier for the analysis of renal survival and a Cox regression model to predict the factors associated with it. Results: 2403 patients were evaluated, stage 3A: 781 (32.5%), stage 3B: 822 (34.2%) and stage 4: 800 (33.3%). Of them, 671 (27.9%) presented rapid progression of CKD. The median follow-up was 3.1 (IQR 1.6 to 4.7) years, 558 patients (23.2%) required starting dialysis and 612 (25.5%) died. Renal survival at one year of follow-up was 98.7% (95% CI 98.1-99.1) and at 5 years 68.3% (95% CI 65.4-70.9). Proteinuria > 3 g/day was associated with rapid progression of CKD OR 12.1 (95% CI 6.6-16.9) and lower renal survival HR 6.6 (95% CI 4.1-10.7). Conclusions: One in three patients with CKD progresses in their disease, the main factor associated with CKD progression and lower renal survival is proteinuria. 1 in 4 patients died and 70% of them did so before entering dialysis.
