Introduction: Occult arterial hypertension is conceptualized as blood pressure within normal parameters when measured at the doctor's office and elevated at home. In Ecuador, this entity has not been studied, nor in the population with type 2 diabetes, and various studies establish that the population with type 2 diabetes has a high risk of macro- and microvascular events. Objectives: Determine the prevalence and associated factors of OHT in patients with DM2, as well as describe the clinical, chemical and sociodemographic characteristics. Methodology: Cross-sectional study with a descriptive and analytical approach that was carried out between the months of January to December 2022 in patients with DM2. ABPM was applied for 24 hours and HTO was diagnosed in individuals with DM2. Laboratory samples were taken, general information was collected through anamnesis, questionnaires and validated tests to stratify possible risk factors associated with patients with OHT. Results: 104 patients were included where HTO was found in 21 participants, constituting 19.4% of prevalence among the population with DM2 at the Teófilo Dávila General Hospital. The factors that were associated with the development of HTO were: BMI greater than 30 or being in grade 1 obesity, family history of high blood pressure, high risk of sleep apnea, physical inactivity and not being an alcohol consumer. Frequent but not significant were: smoking, female sex and ages between 60 and 69 years of age. Conclusion: HTO is a common entity in the population with DM2. The significantly associated factors were BMI, time of diagnosis of DM2, family pathological history of HTN, low level of physical activity, high risk for sleep apnea and not being an alcohol consumer.
