Background: The surgical area was one of the most affected by the COVID-19 pandemic. In Peru, guidelines were implemented, such as reinforcing biosecurity measures and suspending work activities for high-risk personnel and elective surgeries. This, combined with the gastrointestinal manifestations of COVID-19, could have affected the diagnostic concordance of acute abdomen pathologies. Objectives: Describe the concordance between preoperative and postoperative diagnoses and the epidemiological characteristics of acute abdomen pathologies in COVID-19 patients throughout the first year of the pandemic at Hospital Nacional Arzobispo Loayza in Lima, Peru. Materials and Methods: A retrospective and cross-sectional diagnostic concordance research was conducted. Operative reports of patients aged 18 and above with COVID-19 were reviewed for the period from April 2020 to March 2021. Data analysis was performed using STATA/SE 18.0, and diagnostic concordance was assessed using the Kappa coefficient and correlation percentage. Additionally, demographic variables such as age, sex, and pathology frequencies were examined. Results: Data from 110 patients were collected, with the most prevalent pathologies being Acute Appendicitis (61.82%), Acute Calculous Cholecystitis (12.73%), and Bowel Obstruction (7.27%). The mean age was 41.36 years, and there was a predominance of cases in men (57.27%). A Kappa coefficient of 0.8879 and a correlation percentage of 94.85% were obtained. Conclusions: The diagnostic concordance was deemed "Almost Perfect" for the three most frequent acute abdomen pathologies, suggesting that the pandemic context did not compromise diagnostic accuracy in emergency situations.
