Abstract: Antecedente: La etiología genética es muy frecuente en niños epilépticos sin lesiones cerebrales. Sin embargo, aún hay pocos estudios en Latinoamérica. Objetivo: Describir la frecuencia de la etiología genética en niños con epilepsia no sintomática, determinar la frecuencia de las mutaciones, los tipos según número de copias, sus variaciones según edad y género, la comorbilidad clínica y los antecedentes familiares. Material y método: Estudio observacional, realizado en un centro de referencia en Lima. Criterios de inclusión fueron edad menor de 18 años, disponer una historia clínica genética, evaluación neurológica, diagnóstico de epilepsia. Los criterios de exclusión fueron tener causa estructural que origina la epilepsia, ser parte de un síndrome o una enfermedad que incluye crisis epilépticas, falta de un examen genético molecular. Resultados: Fueron revisadas 7257 historias, seleccionados 112 casos y quedaron incluidos 77. Se obtuvieron resultados normales en 5.2% (4/77), variantes de significado incierto en 53.3% (41/77) y variantes patogénicas o probablemente patogénicas en 41.5% (32/77). Conclusiones: La etiología genética en epilepsia en niños fue frecuente, la causa patogénica más común fue la mutación SCN1A, las variantes más frecuentes fueron las de significado incierto. Será necesario realizar más investigaciones por tipo de mutación, tipos de epilepsia y un seguimiento de la evolución clínica de los niños con epilepsia.
Background: Genetic etiology is very frequent among patients with epilepsy without brain injury. Even though, there is lack of information in Latin American region. Objectives: To describe the frequency of genetic mutations in children with non-symptomatic epilepsy, to determine they frequency of the mutations, the variants depending to the age and gender, the types of mutation depending on the variants copy number, the comorbidities, and the family history of epilepsy. Material and methods: This observational study was made in a reference center in Lima. The inclusion criteria were younger age than 18, having a genetic medical record, neurologic evaluation, having the diagnosis of epilepsy. Exclusion criteria were having brain injury that causes epilepsy, to be part of a syndrome or a disease that includes seizures, not having a molecular genetic test. Results: 7257 medical records were revised, 112 of them were selected and 77 were included. Between them 5.2% (4/77) of the cases had normal results, 53.3% (41/77) of the cases had variants of uncertain significance and 41.5% (32/77) of the cases had pathogenic or likely pathogenic variants. Conclusions: the genetic etiology among children was frequent, the most common pathogenic cause was SCN1A, and the most common type of mutation were the variants of uncertain significance. It’s needed further research according to the type of mutation and their clinical correlation, and the follow up of the clinical course of children with epilepsy.
