Background: Using antibiotics for the treatment of bacterial coinfections in COVID-19 patients may lead to significant variation in antibiotic resistance among different pathogens, including E. coli. Objective: To compare the antibiotic resistance of E. coli in urine cultures from patients at Cayetano Heredia Hospital before and after the COVID-19 pandemic, between 2019 and 2022. Materials and Methods: Records of E. coli-positive urine cultures from adult and pediatric patients from all services were included. E. coli susceptibility profile to 22 antibiotics was analyzed based on the records from the Microbiology Laboratory and CLSI guidelines. Results: 5308 records were included (59.4% from 2019 and 40.6% from 2022). Compared to 2019, there was a statistically significant increase in resistance to third and fourth generation cephalosporins, while a statistically significant decrease to aminoglycosides, fluoroquinolones and penicillins/beta-lactamase inhibitors. Specifically, a rise in ESBL E. coli strains (38.3% to 41.4%) was observed, as well as a statistically significant increase in resistance to ceftriaxone (38.7% to 42.3%) and a statistically significant decrease in resistance to ciprofloxacin (61.1% to 54.9%) and gentamicin (27.6% to 21.9%). Resistance to amikacin (1.9%) and nitrofurantoin (3.8%) remained unchanged and continued to be low. Conclusions: Between the pre- and post-pandemic periods, there was a significant increase in resistance to third and fourth generation cephalosporins and a decrease to other antibiotics. Ongoing close surveillance of resistance in E. coli is recommended.
